Looks like a nice specimen of what we used to call "Red-Top Blue/White Agassizii". It is the most common color morph found in commerically supplied A. agassizii from central Peru.
Oliver's name is as good as any. It is descriptive. Descriptive commercial names are "a dime a dozen". Since the Rio Putumayo is the border between Peru & Colombia, it appears that this color morph is more widely distributed than I realized.
