    This is one reason that I quarantine all of my fish for at least 6 - 8 weeks before adding them to an established community tank. Some fish are more susceptible to diseases than others. It's possible that the Black Phantom Tetras introduced something.

    I personally doubt that it was caused by Hexamita because this parasite typically infects cephalic pores. On your fish it occurs where there is no cephalic pore. The, rather high to me, ammonia concentration indicates that the tank's filter can't handle the high population of fish.

    It probably is a male A. steindachneri. I think I see red-orange on the outer margins of the caudal fin, which is indicative of males.

    I think it is your imagination. With 50 years keeping apistos, I've found that fish, no matter if wild or domestic, adapt to water values as long as the fish have been shipped properly (no ammonia damage to gills) and acclimated gradually to values not overly extreme.
