Appistogamma super red cream spot identification

My appistogamma Super red has a cream spot that just showed up on his tail fin. Can anyone tell me what it is and how to deal with it. He is approximately 3 years old.it is only on one side.

3 years is a good age. The fish is old. In human terms it's a senior over 80 years. The immune system is slowly shutting down, cancer coming up... Nothing you can do.
